Patient Rounds: Conduct regular rounds with the medical director and advanced practice provider to assess patient needs and update care plans.
Order Entry: Accurately enter medical orders into the electronic health record (EHR) system.
Medication Reconciliation: Perform thorough medication reconciliation to ensure patient safety and compliance.
Chart Preparation: Prepare and maintain patient charts for regulatory visits, ensuring all documentation is complete and up-to-date.
Care Coordination: Assist with the coordination of care, including various tasks to support patient needs and ensure seamless care delivery.
Transitional Care Management: Support patients during transitions of care, such as hospital discharge to long-term care, to ensure continuity of care and prevent readmissions.
Regulatory Compliance: Support the facility with compliance, ensuring adherence to all relevant standards and guidelines.
Patient Advocacy: Advocate for patients' needs and preferences, ensuring they receive high-quality, patient-centered care.
- Professional & Technical SkillsThis position requires 1 year experience in a clinical setting such as Med/Surg, ICU, OB, etc. and/or Home Health Care and experience in skilled nursing facility.
- EducationGraduate of an accredited School of Nursing. BSN or obtain wihtin 6 years (current ASNs with MHP refer to SPP No. HR-18)

JOB SUMMARYWe are seeking a dedicated and compassionate RN Care Manager to join our long-term care team.
