What are the responsibilities and job description for the Drug and Alcohol Case Manager position at Malibu Wellness Ranch?
Malibu Wellness Ranch in Pike County is a therapeutic equine inpatient rehabilitation program in Milford, PA for those in recovery. We are currently seeking for immediate hire for a Case Manager. Shifts: 8am-4pm.

We utilize the animals on the ranch and horses to help those through the recovery process, while providing them with treatment programs throughout their wellness stay.
Job responsibilities include
- Screen and assess
- Collaborate with other team members to ensure individual needs are met
- Work with local probation, state parole and county correctional facility to ensure individuals' needs are met upon re-entry into the community
- Collaborating closely with drug and alcohol counselors to facilitate discharge planning.
- Facilitate transitional phase of the program for clients, preparing for discharge.
- May arrange medical appointments.
- Perform administrative duties related to a client's care, such as filling out forms for social services benefits and arranging other services as needed throughout the duration of the client's residence in the facility.

One year of experience working with substance abuse clients, preferred. CPR/First Aide certification a plus.
