Malvern Health, Inc. is currently seeking a full time Continuing Care Coordinator to work at our Malvern Treatment Center facility located in Willow Grove. This position is a full time, benefit eligible position.
Position Summary:
Assists patients and their families at Malvern inpatient sites with making a commitment to a full treatment episode, including inpatient, PHP, IOP and GOP. Promotes facilitation with Malverns 90-Day Model of Care, through interactions with patients and their families, seeking both financial and personal assistance in helping their loved one achieve a full episode of care. Maps out aftercare and assists clinical staff with reducing AMA discharges. Assists patients at all levels of care within Malvern with helping them navigate the financial aspects and certification of their healthcare. Acts as a liaison with patients employers in completing the necessary requirements for, but not limited to, leaves of absence from employment and the Family Medical Leave Act. Performs financial counseling for all patients and assists with the Billing Department.
Summary of Essential Position Functions:
- Provides patients access to multi-level teamwork approach to care.
- Quickly responds to priorities established by the Clinical Team, in the day-to-day practice of the position.
- Collaborates with clinical team in making appropriate referrals for continued treatment. Primarily, but not exclusively, coordinating aftercare plans presented by clinical team that include extended treatment, sober living, Medical Assistance cases, and outpatient providers not under the Malvern Treatment Center umbrella.
- Works cooperatively with the Marketing Team to identify priorities for reciprocal referrals.
- Helps patients by providing information, services, and assistance, and participates actively in helping staff to ensure patient safety.
- Maintains a safe, neat and clean environment to facilitate patient safety.
- Conducts interviews with patients and their families, upon request, to facilitate patient stays. Helps patients and their families to navigate the financial aspects of their stays, including leaves of absence as outlined by their employer; COBRA, when appropriate; and the Family Medical Leave Act. Serves as a liaison with union officials, as appropriate to patient stays.
- Ensures with all interactions with patients, their families, their employers and their referral sources that the appropriate standards of confidentiality and release of information are strictly observed and documented.
- In some cases, provides direction and support to patients in signing up for Medical Assistance, as appropriate, and ensuring successful interaction between the patient and the Welfare Office.
- Researches and provides information to patients concerning their eligibility for services such as those provided through the Pennsylvania Office on Vocational Rehabilitation, the Veterans Administration, Medicare and, as appropriate, unions.
- Meets with patients and/or families at the time of admission to explain in detail any and all finances with 72hrs of admission (includes Detox through Outpatient Services and collecting any finances at the time of admission)

Collects co-pays from patients including deductibles, co-insurance and any other out of pocket expenses
- Attends the clinical huddles daily to follow up with any discharge communications, insurance denials, scholarships and payment plans needed
- Attends treatment team weekly to discuss continuing care with inter-disciplinary team
- Collects and scans the credit cards batches each morning from admissions and sends to the Billing Department accordingly
- Establishes any payment arrangements as needed and patient billing practices (includes doing any Financial Hardship Assessments (FHA) that a patient and/or family may ask for any reductions)
- Reviews, maintains and processes fiscal and patient account records and transactions
- Documents all financial communication in the electronic billing software system (includes adding all financial notes and any payments received while a patient is in treatment)

Meets with all patients prior to discharge to close out all financials and finalize payment plans
- If there is a patient who is being referred to a Malvern Outpatient site, paperwork must be reviewed with the patient and signed, scanned and emailed to the Outpatient site
- If a patient is on a payment plan, it is suggested to collect that amount of the payment plan
- Communicates and addresses financial questions related to other levels of care with Malverns continuum of care.
- Helps to maintain a resource directory
- In all interactions, emphasizes the Malvern 90-Day Model of Care in support of the clinical operation.
- Documents all patient and collateral interactions in the electronic medical record.
- Obtains releases of information from patients regarding all communications
- Performs other duties as assigned.

Education and/or Experience: Experience in all aspects of detox and residential care and insurance. Bachelors degree preferred. Three to five years experience in a similar setting. Will consider combination of applicable education and experience.
Technical/Computer Skills: Microsoft Office proficient. Must be detail oriented.
Work Environment and Hazards: Protected from weather conditions. Risk of exposure to communicable diseases. Possible exposure to intoxicated, disruptive and agitated patients.
Physical Requirements: Sedentary work lifting 10 lbs. maximum.
