Our client, a comprehensive, home-based healthcare service, has a need for a Home Health Occupational Therapist in West Allis, WI. The Occupational Therapist assists the physician in evaluating level of function and helps develop the plan of treatment for medically complicated patients after advising and consulting with the patient's family in a home-health setting.
Hours : Monday – Friday 8 : 00am – 5 : 00pm

Are you a graduate of an Occupational Therapy Curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the Council on Education of the American Medical Association and the American Occupational Therapy Association?
Are you currently licensed as an Occupational Therapist by the state of Wisconsin?
Do you have an automobile to use for work along with a license and good driving record?
Are you comfortable treating patients in a home-health setting?
Have you worked with OASIS online charting before?

Job Duties & Responsibilities :
Visit geriatric, developmentally disabled, frail, and / or elderly patients in their home environment (private home, group home, assisted living facility, etc...)
Assist the physician in evaluating level-of-function
Evaluate and establish goals based on the patient’s current level and potential for improvement
Guide and instruct the patient in prescribed therapeutic, self-care, and creative activities that are directed toward improving independence, and physical and mental functioning
Advise, consult, and instruct patients and their families in the use of prosthetic, orthotic, assistive devices and adaptive equipment
Participate in Performance Improvement Activities and discharge planning
Be responsible to ensure the use of the 4Ms (what Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications :
Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the Council on Education of the American Medical Association and the American Occupational Therapy Association
Is currently licensed as an Occupational Therapist by the State of Wisconsin
Active CPR Certification
Must maintain a valid Driver’s License and good driving record
Have and maintain an automobile to be used for work
Has two (2) years of appropriate experience as an O.T. and has achieved a satisfactory grade on a proficiency examination conducted, approved or sponsored by the U.S. Public Health Service
