Home Health Physical Therapist Needed in West Allis, WI – Direct Hire
Our client, a comprehensive, home-based healthcare service, has a need for a Home Health Physical Therapist in West Allis, WI. The PT provides physical therapy services; communicates the patient’s progress with other disciplines, and directs, supervises, and instructs non-professional home health aide staff in the provision of personal care to the patient. The PT will treat medically complicated patients (geriatric, developmental disability, frail, and / or elderly) in a home-healthcare setting (private home, group home, assisted living facility, etc...).
Hours : Monday – Friday 8 : 00am – 5 : 00pm
Job Duties & Responsibilities :
- Under the physician’s order, admits patients eligible for home care services within 24-48 hours.
- Assess and evaluates patient needs / problems, identifies mutually agreed upon goals with patients.
- Reports patient status and need for other disciplines to agency Clinical Supervisor and referring physician.
- Develops patient care plan that specifically addresses identified patient problems and goals. Updates care plans on an ongoing basis
- Provides physical therapist services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care.
- Reports significant findings to patient’s physician and Clinical Supervisor as they occur.
- Re-assess patient for effectiveness related to goals, continuing or discontinuing service, changes to goals, or care plan update.
- Maintains open lines of communications to all members of the continuum of care team.
- Acts as a preceptor in the orientation of new physical therapy staff as requested.
- Participates in discharge planning process per Medicare Criteria and agency policy and procedure.
