JOB TITLE: Adult Day Services Team Leader
DEPARTMENT: Adult Day Services
PURPOSE: Reports to Adult Day Services Manager and is responsible for supervising and assisting individuals served by Manahan in activities and programs as outlined in the individuals’ daily schedules, and assuring active treatment is completed. Acts as a role model for volunteers. Assists with staff supervision in the absence of the Adult Day Services Manager.
JOB FUNCTIONS:
1. Supervises individuals during hours of work. Assists individuals with toileting or other needs throughout the shift as necessary.
2. Responsible for transporting to and from work. Assures that the individuals are clean, neat and appropriately dressed for weather and activities / work. Supervises / assists the individuals with developing their social and work skills.
3. Implements and documents individuals’ programs as scheduled and any other documentation that may be necessary.
4. Encourages individuals to participate in recreational and leisure time activities, and supervises and assists during social or recreational activities / events in the community.
5. Monitors whereabouts of assigned individuals at all times to insure their safety and well-being.
6. Responsible for training new staff & retraining staff on job responsibilities and proper care.
7. Responsible for coordinating and implementing the monthly activity calendar.
8. Assists with staff supervision in the absence of the Adult Day Services Manager.
9. Performs First Aid or CPR as necessary.
10. Uses behavior intervention techniques, as approved by Manahan and as indicated by behavior management programs, as necessary.
11. Completes weekly observation notes for individuals as assigned. Completes daily behavior and program charting for individuals.
12. Identifies situations that require an unusual incident report (UIR) to be written and completes UIRs as necessary.
13. Communicates individuals’ concerns / information to program staff via the staff log.
14. Attends staff meetings and education programs as presented by the company.
EDUCATION:
Bachelor or Associates Degree in Education, Human Services or related field a plus
QUALIFICATIONS:
Experience with job coaching, teaching, or activity planning, implementing and adapting for individuals with developmental disabilities preferred.
Excellent written and verbal communication skills.
Valid driver’s license.
UNUSUAL WORKING CONDITIONS:
Must be available to work in emergency situations.
Must be available to work flexible shifts, days/hours as requested by supervisor.
May be exposed to infectious diseases and behaviorally aggressive individuals.
May be exposed to hazardous cleaning materials/supplies.
LIFTING REQUIREMENTS:
Ability to lift up to 40 lbs.
Work Schedule - full-time/Monday thru Friday - 8am -4pm
