Responsibilities
- Provide direct care and assistance to clients in home community settings, ensuring their safety and comfort.
- Administer medications as prescribed and monitor clients for any side effects or changes in condition.
- Assist clients with daily living activities, including personal hygiene, meal preparation, and household tasks.
- Monitor vital signs and report any significant changes to the appropriate healthcare professionals.
- Implement individualized care plans tailored to each client's unique needs and preferences.
- Engage clients in social and recreational activities to promote emotional well-being and community integration.
- Provide support for individuals with developmental disabilities, helping them navigate daily challenges while fostering independence.
- Maintain accurate documentation of client progress, incidents, and any changes in behavior or health status.
- Collaborate with healthcare providers, family members, and other professionals to ensure comprehensive care.
Qualifications
- Previous experience in home health care or direct support roles is preferred.
- Knowledge of vital signs monitoring and basic medical procedures.
- Familiarity with dementia care practices and strategies for supporting individuals with cognitive impairments.
- Experience administering medications safely and effectively.
- Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- CPR certification is highly desirable.
- Ability to work collaboratively within a team while also being self-motivated.
