Job Summary
We are seeking a Part-Time RN Field Supervisor to work in a fast-paced environment, where time management is essential. The Care Manager position will ensure the recruitment, retention, and development of the best caregivers to ensure quality care to our clients. In alignment with the company mission, vision, and values, the RN Care Manager coordinates, implements, and manages service delivery to provide quality care to clients and their families.
You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.
To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities:
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Develop and implement a written Care Plan for each client, with written approval by the client/ designated family member.
- Able to interact effectively and professionally with clients and/or support network
- Assure that the Client Service Agreement packet is completed for each new client, with the original copy kept for the client file and a copy given to the client/designated family member; collect fees for initial services at the time of assessment or by the first day of service.
- Assure the Care Plan is reviewed on-site, updated, approved, and signed by the client/ designated family member and the appropriate agency personnel.
- Assure that each Caregiver reviews a client’s Care Plan before providing services
- Assure Caregivers are complying with the Care Plan
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications:
- Bachelor’s Degree in nursing, social work, gerontology, psychology, health and human services, or other related field of human services OR 2 years experience in geriatrics and/or care management required
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
