Manitowoc County Human Services Department is seeking a Clinical Case Manager. This position provides and facilitate services for individuals with a mental illness with the goal of avoiding institutionalization and enabling the individuals to live in the community in the most productive and least restrictive manner possible. This position:
- Provides group and individual counseling to mentally ill clients.
- Provides case management, direct services and coordination of services with other providers.
- Assists clients with improving daily living skills.
- Provides crisis intervention services to individuals experiencing mental health or substance abuse crisis situations.
