Purpose Develop, coordinate and implement programs and services for residents with cognitive impairment. Maintain a therapeutic environment to ensure each resident's highest level of well-being for mind, body and spirit.

- Serve as charge nurse during shifts worked when Director of Resident Care and Administrator are not in the building.
- Administer medications per physician's orders.
- Communicate with physicians regarding orders, services and care needs.
- Follow residents' plans of care as prescribed by physicians.
- Document residents' services, changes in condition, exceptions and incidents, as well as follow-up documentation.
- Assist in developing individualized care plans and interventions.
- Collaborate with interdisciplinary teams and families to provide care and services.
- Supervise certified nurse assistants and direct care aides.
- Ensure compliance with state regulations and community standards.
- Collaborate with external professional health service providers.
- Assist in coordinating resident care activities programming.
- Perform other duties assigned by Administrator and/or Director of Resident Care.
Requirements:
- Hold a current unencumbered LPN license.
- Be 21 years of age or older.
- Familiarity with applicable state regulations.
- Read, write, speak and understand English fluently.
- Experience working with older adults.
- Effective communication skills with residents and families.
- Maintain professional conduct with associates.
- Desirable: Supervisory experience.
- Able to perform physical tasks such as lifting, bending and walking without distress.
