Job Summary:
The LPN Wellness Nurse is responsible for assisting and supporting the Resident Services Director with the overall implementation, delivery, and coordination of nursing services in the community. This includes medication administration, direct care and service to residents, teaching and wellness counseling, health promotion, and oversight of Certified Nursing Assistants.
Key Responsibilities:
- Medication administration and management
- Direct care and service to residents, including assistance with daily living activities
- Teaching and wellness counseling to promote healthy lifestyles and habits
- Participation in assessments and development of individualized care plans under the supervision of the Resident Services Director (RN)
- Support of residents and families through effective communication and education
- Oversight of Certified Nursing Assistants to ensure high-quality care
- Training and auditing of care to ensure compliance with regulations and standards
Education/Experience/Licensure/Certification:
- Licensed Practical Nurse currently licensed to practice in the State
- Two years full-time or full-time equivalent clinical experience
- Experience working with older adults and ability to perform tasks that may be physically demanding
- Able to maintain positive working relationships with residents, their families, peers, and other staff members
