Job Title: LPN (Wellness Nurse)
Location: Maplewood at Chardon
Employment Type: Part Time, every other weekend

The LPN Wellness Nurse collaborates with the Resident Services Director to develop and implement comprehensive nursing services.
Key Responsibilities:
- Administer medications
- Provide direct care and support to residents
- Offer teaching and wellness counseling
- Promote health and wellness initiatives
- Conduct assessments under the supervision of the Resident Services Director (RN)
- Support residents and their families
- Oversight of Certified Nursing Assistants
- Training and auditing of care
- Participate in monthly in-service training as required
Education/Experience/Licensure/Certification:
- Licensed Practical Nurse licensed to practice in the State
- Two years of full-time or equivalent clinical experience
- Experience working with older adults
- Able to perform physically demanding tasks up to 50lbs
- Maintain positive relationships with residents, families, peers, and staff members
- In good physical and emotional health
