The LPN Wellness Program Manager provides critical support to the Resident Services Director in executing comprehensive nursing services at Maplewood Senior Living. This position directly impacts the well-being of residents by ensuring timely delivery of health promotions, conducting thorough assessments under the supervision of the RN, and providing overall assistance in training Certified Nursing Assistants.
Main Tasks:
- Assist with assessing residents' health needs and develop individualized care plans
- Collaborate with the multidisciplinary team to ensure effective communication and coordination of care
- Participate in audits to evaluate and improve the quality of care provided
Requirements and Qualifications:
- 2 years of LPN experience, preferably in gerontology or related field
- Strong organizational skills with attention to detail
- Excellent communication skills with ability to collaborate effectively
