In this role, you will collaborate with healthcare professionals, caregivers, and other stakeholders to develop and implement effective care plans that meet the unique needs of each resident. You will also supervise and coordinate the work of Resident Services Department staff, including C.N.A.'s (Resident Services Assistants), to ensure the highest level of care and service delivery.
Responsibilities:
- Develop and implement individualized care plans that address the physical, emotional, and social needs of residents
- Supervise and coordinate the work of Resident Services Department staff, including C.N.A.'s (Resident Services Assistants)
- Monitor and report any significant changes or incidents affecting resident care and well-being
- Participate in ongoing clinical assessments and recertifications to ensure compliance with ALSA requirements
Requirements:
- Licensed Practical Nurse currently licensed to practice in the relevant state(s)
- Basic first aid and CPR certification
- Minimum two years' experience in clinical settings, preferably in assisted living or long-term care environments
- Demonstrated passion for working with seniors and delivering exceptional care
