Job Summary:
Assist in leading the coordination of housing and assisted living services for the Maplewood Manor Assisted Living Community.
Job Qualifications:
- Graduate from an accredited school of nursing with current licensure as a LPN in the state of Minnesota.
- Experience in long term care favored, but not required.
- Preference given to experience in managing staff, administration, or the practice of assisted living services in the residential setting.
- Work effectively with minimal supervision, strong communication skills, and ability to lead a group of people in completing tasks.
- Must meet and maintain requirements of a Home Health Aide.
- Meet the physical job demands of the position.
Hours of Work:
Full-Time position, No scheduled weekends (unless needed).
Responsibilities:
- Works closely with the Clinical Nurse Supervisor/RN to ensure resident plan of care is being followed.
- Works closely with HHA staff, (may include assisting on the floor for up to 32 hours per week or more as needed.)
- Is a champion of Residents Rights and helps create a culture of compliancy.
- Seeks referral inquires and provides tours to potential residents and their families.
- Maintains good relations with physicians, clinics, social worker, etc.
- Participates in an active marketing plan, including planning a calendar of scheduled events to advertise and promote in the community.
- Monitors the ongoing activity program for individual and group activities and participation.
- Supports and serves as appropriate with resident & family council committees.
- Works with ALD to coordinate resident care, including personal, medical, emotional, spiritual, etc.
- Ensure staff follow resident's specific plan of care to ensure highest level of health, safety, and quality.
- Works with ALD to ensures Resident Care Conferences are conducted (min. twice per year) for each resident, or more frequently if needed. Assists & participates with conducting these care conferences.
- Works with LALD to properly report Vulnerable Adult (VA's) to the County/State and/or Ombudsman as needed.
- Ensures any grievance/concern is documented and communicated Community RN and ALD.
- Ensures highest level of resident care is completed with routine visits with residents, family, daily walk-throughs, periodic audits/surveys and timely follow up with concerns.
- Works with ALD and RN to facilitate admissions.
- Maintains resident confidentiality and respect according to HIPPA and the Resident Rights.
- Monitoring and ensure compliance of the process for the residents' personal items being labeled and stored properly.
- Ensure resident dignity and privacy is maintained while providing service to the residents.
- Assists ALD in development and implementation of policies and procedures.
- Works with ALD to assure compliance with all applicable state and federal regulations.
- Works with ALD to assure facility policies and procedures are current and followed.
- Works with ALD to monitors facility census and case mix (acuity) and assure staffing levels are appropriate based on projected staffing.
- Assures supervisory visits are conducted in a timely fashion and documented.
- Assures service plans are current, updated when necessary and changes are shared with Housing Manager for proper billing.
- Develops a good working relationship with the community RN, physicians, county social workers, discharge planners, vendors and other agencies.
- Responsible for insuring ALD and/or RN is notified of changes in resident status, including falls, medication errors, and Vulnerable Adult issues.
- Works with ALD to review vendor contracts and renews as appropriate. Ensures proper documentation is in place.
- Works with ALD to facilitates general orientation for all new hires, ensures department specific orientation is completed as outlined, and schedule is completed as needed and/or directed.
- Assures proper documentation/charges are communicated to billing office in a timely manner each month. Assures any changes or updates are proper communicated.
- Follow safety procedures, policies and manufactures instructions applicable to assigned duties.
- Responsible for prevention of unsafe work situations.
- Expected to lead and attend training and in-service meetings.
- Ensure daily/monthly dietary menu and activity calendars are posted, and food cost budget is followed.
- Works with ALD to adjust staffing levels as census and resident needs fluctuate.
- Has a basic understanding of the information included in the budget, reviews it on a monthly basis to insure expenditures are in line with budgeted amounts and responds appropriately.
- Monitors hours worked, overtime, expenses, spending and has a general awareness and consideration of the finances year to date, and attempts to minimize waste as much as possible.
- Participate in the control of cost-effective services that include proper use of equipment, appropriate use of supplies and effective/efficient organization of resident care duties.
- Ensures residents have an approved payer source; monitors late payments/missed payments and addresses those concerns.
- Responsible to work with ALD and Maintenance for the building to ensure cleanliness, repairs, replacement of fixtures and equipment are addressed timely.
- Develops & participates in disaster plans for the community and conducts and documents monthly drills & yearly severe weather drills according to policies and procedures. Coordinates with maintenance staff.
- Responsible that storage areas are kept neat and organized to promote a safe environment.
- As directed, follow the interview, hiring, and onboarding of new employees process.
- Works with ALD for training, coaching, counseling, and discipline employees related to general customer service and general work performance (i.e. answering phones, attendance, punctuality, respectfulness, following policies and procedures).
- Works with ALD on staff turnover and retention, and to monitor and evaluate unemployment cases.
- Works with ALD for provision of ongoing in-service training for employees.
- Works with ALD to ensure annual in-service training for employees is in compliance with company and state regulations.
- Monitors day-to-day performance of staff as it relates to general work duties and responds appropriately to both strengths and opportunity outcomes.
- Seeks opportunities to maintain education, knowledge and skills.
Salary : $54,080 - $60,320
