Responsibilities
Serves as an advocate the resident in a person centered care environment. The social worker is a part of an interdisciplinary team, responsible for identifying and developing care plans that address the pscho-social needs of the resident/ Acts as a liaison and provides educational support to the residents and their families.
Conducts interviews and ensures that relevant MDS sections and care area assessments are completed in accordance with regulations.
Performs additonal job duties such as care plan meetings, discharge planning, transitional housing transfers, and other resident's needs.
Salary : $22 - $25
