The MDS Coordinator:
- Is responsible for assuring the timely completion of accurate MDS assessments that are in compliance with Federal and State guidelines.
- Maintains assessment schedules within acceptable timeframes.
- Assists with scheduling care plan meetings, ensuring accurate diagnosis are placed in the eMR, and monitoring resident condition changes that may impact MDS assessment requirements.
- Provides education and guidance for nursing staff regarding MDS completion and required documentation.
Qualifications:
- RN with current NYS license
- 2 years of LTC experience
- Current experience in MDS 3.0, Medicare Reimbursement regulations, and PDPM payment system preferred.
