Summary: Social Services Director plays a crucial role in providing comprehensive social services to residents and their families within the Mayflower Skilled Nursing Facility and Rehabilitation Center. They are responsible for assessing the psychosocial needs of residents, collaborating with interdisciplinary teams, and facilitating communication and support between residents, families, and the facility staff. The SNF Social Worker contributes to the overall well-being and quality of life of residents by addressing emotional, psychological, and social needs.
Essential Functions:
Job Duties:
- Conduct thorough psychosocial assessments of residents upon admission and periodically thereafter to identify their emotional, social, and psychological needs.
- Collaborate with residents, families, and healthcare teams to develop individualized care plans that address the residents' social and emotional well-being.
- Provide counseling and emotional support to residents who may be experiencing challenges related to health, loss, adjustments, and other psychosocial issues.
- Offer coping strategies and resources to residents and families to promote emotional resilience.
- Serve as a liaison between residents, families, and facility staff, facilitating effective communication and addressing concerns or needs that may arise.
- Advocate for residents' rights and preferences, ensuring that their voices are heard in the decision-making process.
- Collaborate with the interdisciplinary team to develop discharge plans, ensuring a smooth transition to home, another facility, or a higher level of care.
- Provide support and resources for residents and families during the transition process.
- Offer guidance, information, and resources to families to help them understand the resident's needs and adjust to changes in the care plan.
- Assist families in navigating the healthcare system and accessing relevant community services.
- Provide immediate support and intervention in situations of crisis, such as emotional distress, family conflicts, or end-of-life concerns.
- Maintain accurate and up-to-date documentation of assessments, care plans, interventions, and interactions with residents and families.
- Collaborate with facility staff, including nurses, physicians, and therapists, to ensure a holistic approach to resident care.
- Provide education to staff regarding psychosocial issues, communication strategies, and resident-centered care.
- Ensure compliance with state and federal regulations related to resident rights and social services in a skilled nursing facility.

Communication
- Communicates effectively with supervisor and other staff
Education/Experience/Licensure/Certification
- Master's degree in Social Work (MSW) from an accredited institution.
- Licensure as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Certified Social Worker (LCSW) in Massachusetts.
- Knowledge of state and federal regulations related to skilled nursing facilities and resident rights.
- Excellent communication, counseling, and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary team.
- Empathy, compassion, and a strong commitment to improving the lives of residents and their families.
Physical Requirements
- Physically able to bend and reach.
- Physically able to push and pull and lift up to 60lbs at times.
- Physically able to stand for extended periods of time
- Could be required to use a Hoyer Lift
Miscellaneous
- Required to work weekends and holidays as assigned
- May be required to work on shifts other than the one for which hired
- May be required to work extended hours up to 16 hours per day
- Subject to exposure to infectious waste, diseases, conditions, etc. including HIV, AIDS and Hepatitis B & C viruses
- Could be subject to hostile and emotionally upset residents due to mental status
- Background, criminal and drug test required
- Could be asked to submit to random drug test during employment
