Duties/Responsibilities
- Uses 2 patient identifiers before giving treatments, illustrates process.
- Demonstrates awareness of patient rights.
- Uses respect & dignity when dealing with death & dying.
- Reassesses patient in response to changes in condition after intervention & as needed, & documents in EHR.
- Provides treatments as ordered in a timely & appropriate manner & documents in EHR.
- Provides education to patients & family & documents in EHR in a timely manner.
- Conducts assessment of patient.
- Conducts initial visit including detailed addiction history & education about the nature of addiction & the goals & expectations of treatment.
- Consults with patient & medically assisted treatment provider about treatment options & preferences; coordinates initiation of treatment plan.
- Monitors patient closely (in-person or by phone) for changes in severity of symptoms & medication side effects; educates patients about medications & medication side effects, as needed; encourages treatment adherence.
- Uses behavioral activation techniques with patients as an adjunct to other treatments.
- Provides optional evidence-based, brief structured psychotherapy (e.g., PST-PC).
- Participates in regular caseload supervision with Clinical Psychologist, HSPP.
- Coordinates & facilitates communication between patient, medically assisted treatment provider, Psychiatrist; provides recommendations for change in treatment plan according to evidence-based algorithm & expert supervision; supports implementation of new plan.
- Documents all encounters according to organizational policies & procedures in the EHR & monitors outcome measurements.
- Facilitates treatment referrals, as needed.
- Completes relapse prevention plan with patients who are in remission.
- Demonstrates knowledge of child abuse, endangered adult abuse, & domestic violence; has ability to identify suspected abuse & report according to legal guidelines.
- Demonstrates ability to develop rapport with patients & families with diverse cultures, values, & life experiences.
- Provides education to individuals on how systems work & how to access services.
- Provides updated, comprehensive resource list to meet patient needs.
- Serves as an advocate on behalf of the patient to access services.
- Provides education to patient & family & documents in electronic health record in a timely manner.
- Strong customer service skills & team player.
- Effectively communicates both verbal & written.
- Able to multi-task & prioritize.
- Skilled in interpersonal relations (i.e. interviewing, assessing, developing discharge plans with interdisciplinary team, implementing discharge plans with physician & patient’s family, community agencies, & referral sources.
- Able to operate basic office equipment & hospital applications.
- Master’s degree majoring in Social Work.
- Licensed Clinical Social Worker, LCAC, LAC, or LMHC in the state of Indiana.
- Minimum 1 year relevant work experience.
- Frequent walking, standing, reaching, use of hands & talking; occasional sitting, pushing, pulling, climbing, stooping, kneeling & manual dexterity.
