Job Summary:
To assist with the collection, monitoring & reporting of population health & quality data for participation in governmental & commercial insurance quality programs & serve as a liaison between physician services & the quality department to help ensure Margaret Mary Health practices meet quality & population health goals.
Supervisory Responsibilities: Oversees daily operations of Population Health team including Care Coordinators and AWV RN’s.
Duties/Responsibilities:
- Uses only approved abbreviations.
- Understands patient safety goals & actions taken.
- Uses 2 patient identifiers when necessary.
- Reports sentinel events or incidents as warranted.
- Have working knowledge of quality programs which Margaret Mary Health participates and works in collaboration with Quality department on meeting program requirements.
- Works in collaboration with various departments within Margaret Mary Health, including but not limited to inpatient units, emergency department, and specialty practices to promote population health efforts.
- Monitors practice quality metrics and communicates findings with Population Health Medical Director and Population Health team.
- Assist practices with collection of required quality & population health data.
- Reports population health data in required format.
- Serves as liaison with practices, third party payors, & quality department.
- Educate providers & staff regarding documentation needed to meet quality measures & maximize HCC coding.
- Provide feedback on current processes & procedures to meet & improve population health goals.
- Attends Care Coordinator and AWV team meetings, quality huddles, & ACO meetings.
- Serves as the ACO Champion and represents Margaret Mary Health as such in ACO meetings.
- Coordinates patient outreach efforts where needed to meet population health goals.
- General oversight of schedule & daily tasks of Population Health team to ensure proper completion & utilization as appropriate.
- Addresses staff concerns and escalates to Manager/Director as needed.
- Assists in coordination of orientation for new staff.
- Evaluates, supports & encourages all customer service improvement efforts.
- Liaison & coordinator for EHR updates.
- Facilitates daily huddles & assignments.
- Evaluates & assists with implementation of workflows.
- Completes timecards and assists with coordination of pto requests.
- Assists with hiring process & training personnel, as required, to ensure efficient & effective office work flow, according to HR & office policy.
- Maintains personnel files for each team member; license, certifications, CPR, & phone numbers, etc. are up-to-date.
- Monitors team member’s compliance with quality improvement plans & measures, timely charting and charging, and accurate documentation.
- Responsible for team member performance evaluations in collaboration with the Practice Manager/Director.
- Maintains schedule to ensure coverage, including call-ins, PTO, leaves, etc. optimizing staff resources; monitoring overtime and worked hours.
- Monitors patient satisfaction survey results and follows up, as needed.

Education and Experience:
- Minimum Associates degree in nursing, Bachelor’s degree preferred
- Minimum of 1 year relevant work experience required, Care Coordination experience required
- Must be licensed as a Registered Nurse in the state of Indiana
Physical Requirements:
- Frequent walking, standing, pushing, pulling, stooping, kneeling, reaching, talking, & use of hands
- Occasionally may need to lift up to 25 lbs
