32Job Description Summary:
The Diabetes Care Program Coordinator will work with both the outpatient and inpatient diabetes care program teams under the supervision of the Diabetes Care Program manager. The Diabetes Care Program Coordinator will lead various projects, department improvements and day-to-day operations delegated by the manager. These include but are not limited to:- Front office support
- Clinician/provider support
- Data collection for the inpatient Joint Commission certification, outpatient American Diabetes Association recognition and Diabetes Prevention Program (DPP)
- Manage details and provide support for Diabetes Prevention Program and diabetes self-management education (DSME) group classes, along with clinical leader(s)
- Assist with student intern and volunteer program where applicable
- Other duties as assigned
Job Requirements, Prerequisites and Essential Functions:
EDUCATION
1. Bachelor’s degree (BA or BS) preferred.
EXPERIENCE
1. Two years’ experience in clinical back-office support preferred.
2. Two years’ experience in customer service preferred, preferably in healthcare.
LICENSURE AND CERTIFICATIONS
1. Basic Life Support through an American Heart Association (AHA) Accredited Course required at hire.
2. Integrative Agitation Management (IAM) Required within 30 days of hire.
3. Certified Medical Assistant preferred.
PREREQUISITE SKILLS
1. Adept at developing and implementing process improvements
2. Skilled in word processing, excel spreadsheets, power point, electronic health record(s)
3. Demonstrates knowledge of current American Diabetes Association (ADA) requirements for DSME and DPP
4. Demonstrates ability to organize and prioritize in a rapidly changing work environment
5. Demonstrates oral and written skills
6. Demonstrates computer skills
7. Demonstrates presentation skills
8. Demonstrates ability to work collaboratively and develop positive interactions with patients, families, physicians, staff and other disciplines
9. Ability to read, write, understand and follow English verbal and written instructions
10. Ability to set priorities, achieve objectives, solve problems, meet deadlines, coordinate simultaneous projects, and make decisions
11. Demonstrates ability to work independently and with a team.

The compensation for this role listed on this posting is in compliance with applicable law. The selected candidate’s compensation will be determined based on the individual’s skills, experience, internal/market equity factors, and qualifications. This posted minimum and maximum range represents the minimum and maximum of what we reasonably expect to compensate for the position. Furthermore, all compensation decisions are ultimately determined in accordance with our compensation philosophy. Compensation for positions covered by collective bargaining agreements are governed by the agreements in the aforementioned document.
