What are the responsibilities and job description for the Community Integration (CI) Liaison position at Marion County Services Inc DBA Abilities?
RESPONSIBILITIES:
- Supervise Community Integration staff.
- Provide daily guidance in implementation of individual support plans.
- Complete performance plans and evaluations for all Community Integration staff. Present employee performance evaluations to his/her supervisor for review and signature.
- Provide direct Community Integration services as needed.
- Attend Individual Support Plan conferences as needed for individuals to provide input and gain information concerning individuals needs, progress, and possible changes required.
- Conduct monthly meetings with Community Integration staff.
- Meet with PM weekly, or as deemed necessary by PM, for updates.
- Monitor individual progress.
- Conduct visits to homes of individuals living independently as needed.
- Direct individual and family contact and assistance. Maintain records of contacts and follow up activities.
- Assist individuals with applications for services from other agencies and other paperwork required.
- Assist CI individuals in budgeting as needed.
- Serve as Liaison for already established community based programs.
- Assist with individual inclusion into the community of activities involving individual.
- The CI Liaison is required to meet with his/her supervisor on a weekly basis. The following needs to be accomplished at these weekly meetings: ? Schedule turned in and discussed if any issues. ? Activity plans reviewed with supervisor. ? Any issue that has arisen with program staff discussed and ultimately resolved.
- Write an article for the newsletter.
- Assist individuals and attend the Friends Council meetings.
- Perform all other related work duties assigned.
