Job Summary: The Client Care Coordinator plays a vital role in our mental health intensive outpatient program, ensuring smooth coordination between departments, managing scheduling and referrals, supporting intake processes, facilitating group sessions, and maintaining accurate documentation. This position also handles office upkeep and provides outstanding customer service to create a welcoming and supportive environment for clients.
This is an in-person position Monday-Friday 1pm-9pm schedule.

Key Responsibilities:

  • Care Coordination & Administrative Support:
  • Assist with scheduling, referrals, and intake documentation.
  • Facilitate communication between departments to enhance client care.
  • Maintain organized records and ensure compliance with HIPAA and program policies.
  • Stock office supplies and maintain a clean, organized work environment.
  • Provide excellent customer service to clients, families, and staff.
  • Coordinate meal services, including ordering and scheduling lunches/dinners.
  • Behavioral Health Support:
  • Lead group therapy sessions each week as assigned (evenings included)
  • Conduct facility rounds, ensuring compliance and safety.
  • Track and update logs (freezer, fridge, sanitization).
  • Assist with client de-escalation when necessary.
  • Attend team meetings, supervision sessions, and quality assurance (QAPI) meetings as assigned.
  • Case Management Responsibilities:
  • Coordinate with therapists, psychiatrists, and external providers for seamless care.
  • Facilitate referrals to community resources (housing, employment, healthcare).
  • Assist in crisis intervention, ensuring safety plans are in place.
  • Maintain accurate case notes and ensure regulatory compliance.
  • Team Collaboration & Communication:
  • Communicate effectively with clients, families, and colleagues.

Qualifications & Requirements:

  • Education: High School Diploma or equivalent required; Associate’s or Bachelor’s degree highly preferred.
  • Experience: Prior experience in healthcare, behavioral health, or administrative support preferred.
  • Strong organizational, multitasking, and problem-solving skills.
  • Excellent verbal and written communication abilities.
  • Proficiency in Microsoft Office and electronic medical records (EMR) is required
  • Ability to work collaboratively in a fast-paced environment.
  • Commitment to client-centered care and professionalism.

Work Environment

  • This job operates in a fast-paced clinical and office environment, requiring frequent interaction with clients, medical providers, and administrative staff.
  • Requires frequent movement throughout the facility and use of standard office equipment.
  • Occasionally required to lift/move objects up to 25 lbs.

Job Type: Full-time

Pay: $50,000.00 - $60,000.00 per year
