Location: Reedley, CA
Schedule: 8:30 am – 5:00 pm/ Monday – Friday (may need to work an occasional evening or be available to meet with client during the weekend, if necessary.
Pay Rate: $25.00 (entry level) - $34.30/hourly (depending on experience)
Reports To: Community Resources Manager
(This position is dependent on grant funding)
SUMMARY: To provide comprehensive Crisis Response and Case Management services as it relates to clients seeking services. This position will also provide case assessment, case plan development, client support and case monitoring, for clients seeking and obtaining MMC services due to Domestic Violence. The Case Manger provides residential & non-residential clients direct case services including advocacy, support, goal setting, assistance in identifying options, evaluation of needs and information and assists the clients in making necessary community linkages to support their self-sufficiency goals. The Case Manager maintains client interaction utilizing Trauma Informed Care practices which support and identify immediate needs, issues or concerns of clients. Communicate effectively with community agencies and law enforcement to ensure satisfaction with services provided.
The Marjaree Mason Center is 24/7 operation and all employees, at any point of time, may be asked to perform other work duties from those normally assigned such as answering the hotline or working in the shelter.
Duties and Responsibilities:
- Manage a full case load of residential & outside clients who are DV survivors in need of ongoing services coordination, resources and supports;
- Answer crisis calls, provide information on services, make referrals by phone, and/or route client calls as appropriate;
- Interact and support the assessment process of DV crises, in coordination with the MMC Crisis Response Team;
- Collaborate and interact with LE departments regarding DV crisis clients seeking services;
- This position will complete crisis drop in assessments of service needs and provide supportive services to assist a drop in crisis person, assisting them into the correct system of care if not MMC;
- Make informed decisions regarding client entry into the MMC Emergency safe house, based on clients safety need;
- Facilitate DV transfers from other housing programs, providing those victims/families safe housing until alternative long term safe housing arrangements are made;
- Work in conjunction with homeless service providers in our area, to ensure access and services for homeless entering our facilities;
- Maintain positive community relations with other agencies, and law enforcement, and advocate for client services;
- Maintain a cooperative relationship with Safe House and Crime Victims Assistance Center (CVAC) programs for clients we are jointly working with;
- Responsible for the Intensive coordination and communication within the coordinated entry system process; and collaborate with internal and external entities in order to deliver more efficient use of resources;
- Works cooperatively with the agency clinical staff and the Children’s Enrichment Services Coordinator to maximize services, assessments and resource development for children in residence;
- Develops plan of cares/case plans that will focus DV survivor adults, families, and children toward self- sufficiency and achievement of meeting their safety, legal, clinical and housing needs during resolution of the DV situation;
- Work within required project guidelines and follows agency policies and procedures.
- Participates in programmatic development and contribute to the ideas pertaining to the enhancement end efficiency of service delivery;
- Continuous safety planning with clients for current and future wellbeing.
- This position requires the use of personal vehicle when agency vehicle is not available, as this position requires regular mobile advocacy and involvement throughout the Fresno County;
- Meets with the clients on a regular basis to monitor goal achievement, identify/address barriers to service and to make ongoing referrals for needed services;
- Support child/parent activities focused on building positive family relationships.
- Coordinate with the emergency Safe House, clinical staff, and legal services, as well as other areas of the MMC organization, to insure appropriate client referrals and participation in programs;
- Provide case management advocacy & support in necessary meetings for clients (court, TDMs etc.);
- Keep case files maintained, up to date with case plans and programming that meet the developed project implementation plans-as per the agency requirements;
- Complete and maintain required documentation current for agency and grant needs, including HUD ESG forms and other grant client service documents that require reporting via HMIS and in support of other agency statistical requirements utilizing Apricot database;
- Responsible for the enrollment data entry for all new intakes into the Safe House, and process of transfer of clients into the Safe House or temporary residence as appropriate;
- Work with Supervisory and management staff to achieve programmatic expectations, implementation and development to meet/maintain compliance with grant requirements;
- Participates in agency coordination, training, and service enhancement activities.
- Participates in case reviews and family assessments as required;
- Other duties as assigned and necessary to achieve agency, programmatic, and client goals;
- Perform miscellaneous duties to support the Reedley office;
- Will require the use of personal vehicle with mileage reimbursement provided;
- Works within program guidelines and follows agency policy and procedures;
- Regular attendance and punctuality are considered part of each employee’s essential job functions.

Requirements
EDUCATION:
- Must have completed 30 units of post-secondary work or obtained an Associate’s degree.
- Knowledge, abilities and skills required to be successful in the position:
- Bilingual in Spanish.
