What are the responsibilities and job description for the WELLNESS COACH - Adult CPR position at Mark Twain Behavioral Health?
The Wellness Coach supports CPR clients in a collaborative process to assess the personal wellness of the client, identify strengths and wellness dimensions to improve, create a plan and offer support to achieve the wellness goal. Through specialized training, the Wellness Coach helps clients to make positive changes on health issues through the use of knowledge of health and wellness, health literacy, effective communication skills, coaching principles and strategies to motivate and inspire. Wellness Coaches are expected to work in a variety of settings including agency offices, consumer homes, local human services offices, as well as local residential care facilities.
ESSENTIAL DUTIES & RESPONSIBILITIES:
A. Utilize a deliberate, intentional process to assist the client in identifying and reaching selected wellness
goals, particularly in the physical wellness dimension.
B. Establish relationship and orient new clients to wellness coaching
C. Assess strengths and area of focus for improvement
D. Assist client to create SMART goal and wellness plan
E. Provide accountability with routine check-ins
F. Engage in brainstorming and empower client to identify individual solutions to health problems and
barriers
G. Monitor client participation and progress
H. Ensure communication with CSS regarding Wellness Plan
I. Complete Wellness Planning Tool in Carelogic
J. Maintain clinical records to comply with Wellness Coaching standards
K. Participate in development of treatment planning with addition of Wellness SMART Goal in
conjunction with consumer, family members and other stakeholders.
L. Complete required documentation in a timely fashion, including treatment plan and progress notes in
agency-mandated format.
M. Assure all supporting documentation is present in each client file
N. Serve as client advocate as needed
O. Behave in a professional and courteous manner with co-workers, supervisory staff, community
members, as well as consumers of behavioral health services and their families.
P. Complete daily schedule in Carelogic
Q. Produce billable hours over 50% of hours worked
R. Adhere to all client rights, including confidentiality
S. Other job duties may be assigned
QUALIFICATIONS:
This position requires a bachelor’s degree in a Human Services field or four years of equivalent experience with consumers and their families receiving psychiatric, substance use, or developmental disabilities services OR a Registered Nursing license with at least two years of full-time equivalent psychiatric experience. Current Cardiopulmonary Resuscitation, First Aid, and Wellness Coaching Certification are required. Medication Aide, Level I Certification is also required.
