Position: Social Worker Department: Hospice Location: Marlette, MI Hours: Full Time. Days. Full Benefits. Aspire Rural Health Systems is seeking a full-time social worker for our Hospice care and CRTN Cancer Center. We are looking for those with a great attitude to join our dedicated team of healthcare professionals constantly striving to provide the highest quality of services for our patients. REQUIREMENTS:

• Master's degree in Social Work (MSW) from an accredited program preferred, Bachelor's Degree will also be accepted
• Minimum of 2 years of experience in healthcare or hospice settings preferred
• Interpersonal skills: supportive, active listener, effective communicator with patients, family members, medical and hospital staff, ability to be both a team member and a team developer
• Conciliation/facilitation skills; analytical; problem solving skills

RESPONSIBILITIES: This position is responsible for serving as a liaison between patients and hospital staff, assisting patients in interpreting hospital policies, procedures, and services and in resolving problems and concerns. Monitor patient satisfaction and contribute to the continuous improvement of overall hospital and health service operations. Assist in safe discharges for patients including swing bed.
