As a Resident Care Manager (RCM) - RN
Looking of for a Nurse to join our team as our Nurse case manager for our small Memory care unit that is located within a SNF Skilled Nursing Facility setting. Are you a Nurse with passion for Alzheimer's, Dementia, and related disorders conditions? Do you want to make a difference in the lives of others? Your experienced nursing skills will be utilized in t our Five-Star Resident-Centered quality of care facility working closely with residents, their families, front line nursing staff, Physicians, healthcare providers and the facility inter-disciplinary teams.
