Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop personalized care plans.
- Administer medications, perform wound care, and provide other skilled nursing services in accordance with physician orders.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure coordinated and effective care.
- Educate patients and their families on managing health conditions and promoting a healthy lifestyle.
- Monitor and evaluate patients' progress, adjusting care plans as necessary.
- Maintain accurate and up-to-date patient records in accordance with regulatory standards.
- Provide emotional support and compassionate care to patients and their families.
- Current and valid RN license in New York State.
- Minimum of 1 year of nursing experience, with a preference for home care or community health experience.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
- CPR certification.
- Reliable transportation and a valid driver's license.
