As a Home Care Pediatric Physical Therapist, you will play a vital role in improving the lives of children by providing specialized physical therapy services in the comfort of their homes. Responsibilities :
- Conduct thorough assessments of pediatric patients to identify physical therapy needs and develop personalized treatment plans.
- Implement evidence-based physical therapy interventions to enhance mobility, strength, coordination, and overall functional abilities in pediatric patients.
- Collaborate with interdisciplinary healthcare teams, including physicians, nurses, and other therapists, to ensure comprehensive and coordinated care.
- Educate and involve parents / caregivers in the therapeutic process, providing guidance on home exercises and activities.
- Monitor and document patient progress, adjusting treatment plans as necessary to achieve therapeutic goals.
- Participate in case conferences and team meetings to discuss patient progress and develop integrated care plans.
