SUMMARY
The Lead Support Staff provides direct care to persons who are deaf/hard of hearing and also diagnosed with developmental and/or physical disabilities. They will implement service plan and other habilitation programs. They will facilitate and promote community integration, and will assist in day-to-day site management with minimal supervision. They will orient and schedule Supportive Living Service Support Staff.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Ensures individuals’ health issues are addressed. This may include scheduling doctor’s appointments, providing transportation to appointments, following through on doctor’s orders and communicating all pertinent health issues to supervisor.
Monitors, observes and prompts individual’s self-medication administration.
Assists individuals in daily activities including daily hygiene functions.
Implements behavioral goals, teaching strategies, monitoring procedures, exercise programs and other team recommended programs and procedures.
Communicates with Management staff daily through telephone, voice mail, and/or written communication.
Completes office related tasks during office hours when supporting the Supportive Living Services Case Manager Day to day site management with minimal supervision.
Orients and schedules Supportive Living Service Support Staff Assists in the completion of individual assessments.
Models positive adult roles in the community.
Essential employee – assists in scheduling trained staff to fill in shifts for this position during scheduled time off due to the essential function of this position.
Supervises and assists in the preparation of food for individuals following special prescribed diets and food consistency. Implements eating procedures as written, if applicable.
Operates motor vehicles safely and in accordance with State Law (agency and personal automobiles) while on work related duties. Will be required to drive own vehicle for work.
EDUCATION and/or EXPERIENCE
High school diploma or general education degree (GED); 1 year of successful human services experience in related position. Fluency in American Sign Language strongly preferred.

Department of Developmental Services - medication certification
CPR and First Aid certification
PMT certification

SALARY
$20.50/hr increase to $20.97/hr after 6 months.
