Position Summary:
Social Worker/Case Manager/Counselors: Non-profit agency is seeking a dedicated and compassionate worker to provide clinical treatment for clients with dual diagnoses. The ideal candidate will have a strong background in mental health and addiction treatment, and the ability to build supportive and trusting relationships with clients and their families. Individual should be experienced with substance abuse to conduct screening and assessments; provide individual and group counseling; develop treatment plans; create and maintain client charts in compliance with regulatory agencies and funder; facilitate relapse prevention and continuing care services; and provide follow-up and aftercare counseling and case management. Key Responsibilities:
- Conduct ongoing assessments of clients' addiction symptoms and treatment responses.
- Adjust treatment plans as necessary to address changes in clients' behaviors.
- Provide substance abuse and dual diagnosis education to clients and their families.
- Deliver direct clinical services, including individual therapy, group therapy, and family therapy, using a non-confrontational, client-centered approach.
- Coordinate and manage case activities for an assigned group of clients.
- Develop, implement, evaluate, and revise treatment goals and plans collaboratively with clients and the individual treatment team (ITT).
- Educate and support clients' families, advocating for clients' rights and preferences.
- Great communication and interpersonal skills when dealing with participants, staff, community partners, and stakeholders.
- Conduct holistic assessments with team members.
- Serve as a liaison and consult with community agencies and families to maintain coordination in the treatment process.
- Participate in on-call rotation for crisis intervention/support, available 24/7.
- Document client progress according to established methods and procedures.
- Collect, monitor, interpret, evaluate, and utilize data to improve functioning, retention, and enhance productivity and outcomes.
- Have experience in different interventions which include, but are not limited to; psychodynamic, cognitive behavioral, harm reduction, strengths based, motivational interviewing and mindful-based practices.
Non-Essential Functions:
- Occasional interstate travel.
- Perform other related duties as assigned.

- Knowledge of community resources and services is preferred.
Qualifications Required:
- Specialized Experience: One year of specialized experience which includes assisting with clinical assessment of substance abuse patients, assisting with formulating treatment plans or discharge plans, and assisting with providing counseling to substance abuse patients.

Combination of Education and Experience: A combination of education and experience may be used to qualify for this position as long as the computed percentage of the requirements is at least 100%. To compute the percentage of the requirements, divide your total months of experience by 12. Then divide the total number of completed graduate semester hours (or equivalent) beyond the second year (total graduate semester hours minus 36) by 18. Add the two percentages.
Preferred Candidates:
- Master’s degree in a relevant field
- License (LADAC, LLCSW, LSW, LICSW), or be working towards obtaining a license to work in the field
- Post-secondary education and training, including internships and supervised practical experiences in clinical or rehabilitation settings with individuals with severe mental illness and addiction disorders.
- Commitment to ethical standards and continuous professional development.
- Ability to establish supportive, trusting relationships with clients, their families, and their networks.
