JOB SUMMARY
The Registered Nurse Care Coordinator is responsible for ongoing case management coordination through the continuum of care. Responsible for the clinical program components, provide expertise regarding clinical services needed with a focus on preventive and primary care; and provide disease case management where relevant.
The nurse uses motivational interviewing techniques while lending support in the efforts in managing complex patients. Patient population ranges from chronically ill to healthy individuals wanting to prevent disease. The nurse works with the patient to develop a plan and facilitates patient's access to appropriate health care services and community resources. Patient self-management and increasing patient knowledge of their disease process are other foci for the nurse. The nurse is accountable for the management of patient's therapy enrolled in case management. The nurse supports the physician's practice by following physician protocol to manage patient's therapy, related education, triage and coordination of care and referrals when appropriate.
In addition to working with the patients and their families, the nurse interacts regularly with physicians and personnel from within the Marshfield Clinic Health System (MCHS) and outside the system to coordinate care of patients.
JOB QUALIFICATIONS
EDUCATION
Minimum Required : Graduate of an accredited school of nursing.
Preferred / Optional : Bachelor of Science in Nursing (BSN), bilingual in English and Spanish, basic knowledge of epidemiology.
EXPERIENCE
Minimum Required :
A minimum of three years’ recent clinical experience in hospital or clinic, working with adults.
Experience with patient interaction on the telephone.
Experience with computers.

CERTIFICATIONS / LICENSES
Minimum Required : Current State of Wisconsin Registered nurse License or Nurse Compact License. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred / Optional : Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
