JOB SUMMARY
The Social Worker (BSW) – Home Health / Hospice functions as an integral member of an interdisciplinary team that provides supportive services to help meet patient, family, social, emotional and economic needs in relation to the patient’s diagnosis. Social Worker (BSW) – Home Health / Hospice provides psychosocial support for patients, collaboratively working toward program objectives of achieving the best possible quality of life for patient and family. The Social Worker (BSW) - Home Health / Hospice provides services and care in accordance with a comprehensive patient care plan and under the supervision of a Social Worker (MSW) – Home Health / Hospice.
JOB QUALIFICATIONS
EDUCATION
Minimum Required : Bachelor’s degree in social work or related field.
Preferred / Optional : Master’s degree in social work.
EXPERIENCE
Minimum Required : One year of experience in a healthcare setting.
Preferred / Optional : Three years’ experience working as a Social Worker working with a target population, community health setting or in a healthcare facility working with children and adults.
CERTIFICATIONS / LICENSES
Minimum Required :
- Current Social Worker license to practice without restriction in Wisconsin.
- Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
- Current applicable state driver’s license with acceptable record.
Preferred / Optional : Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
Work Schedule
Flexible daytime hours; 4 to 5 days per week
