Summary: Under the supervision of the Director of Health and wellness/Therapeutic Services, the Physical Therapist will carry out an individualized and group program of physical therapy – including evaluation, treatment, program planning, implementation, education, and communication – to maximize Resident progress toward achieving and maintaining the best ability to function. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff. Supports the Corporate Compliance Program and the Code of Ethical Conduct and promotes the mission and values of Martha Jefferson House both internally and within the community.

Supervisory Responsibilities
- Provides supervision to Physical Therapy Assistants according to the State Board of Physical Therapy guidelines.
- Provides supervision to Health & Wellness Technicians per the Director of Health & Wellness/Therapeutic Services.
- Provides direct Resident care, evaluates functional needs and outcomes, and consults with other specialists. Review resident functional needs and adjust care plans to ensure optimal Resident outcomes.
- Designs and implements a plan of care for Residents based on a thorough assessment of potential development and incorporates participation by other significant parties whenever appropriate.
- Conducts therapy practice within defined standards of care.
- Communicates effectively with other professional and support staff to achieve positive Resident outcomes.
- Practices MJH Community policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
- Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains orders and signatures.
- Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
- Provides direction and leadership to staff and acts as a resource to promote physical therapy in the MJH Community.
