Job Description: Family Resource Center Family Partner
Reports to: FRC Division Director
Classification: Non-Exempt
Department: 700 – Family Resource Center
SUMMARY: Under the guidance of the Division Director and IWYC/FRC Program Coordinator, the FRC Family Partner collaborates with the FRC Clinician to conduct FAST Assessments and, when necessary, create Family Support Plans for family members facing CRA-related challenges. The Family Partner is responsible for overseeing the implementation of these plans, with support from other FRC staff, and assists families in accessing information, resources, referrals, and required forms to receive services. Flexibility for evening and weekend work is required. This position may be eligible for a Flexible Work Arrangement, subject to approval by the Division Director, CEO, and the completion of a Flexible Work Agreement.
Job Responsibilities and Essential Functions (essential functions*):
- Provide Services to Families:
- Provide information and referrals to community resources and completes necessary forms for service for individuals seeking support from the FRC*
- Maintain and continually track data as it relates to providing services to FRC families*
- Assists Clinician in developing the Family Support Plan for family members experiencing CRA-related issues.
- Oversees implementation of the Family Support Plan*
- Attends trainings as requested by the Department of Children and Families and the Program Director
- Attend meetings as requested by the Department of Children and Families either via Zoom or in person.
- FRC Programming:
- Assist in presenting evidenced-based FRC program curriculum as well facilitate other programming of the FRC, including groups, events and community events*
- Attend trainings relevant to this position
- Other:
- Participate in at least two initiatives of the MVCS Quality Improvement Team per Fiscal year
- Attend the MVFC Council for Young Children Quarterly Meetings as a representative of the Family Resource Center
- Act as a Liaison between the FRC Office and the Cape and Islands Department of Children and Families working to foster collaboration and easy access for services via referrals for families involved with DCF.
- Attend off island meetings as deemed necessary by the Program Director
