JOB SUMMARY:
The Outreach and Triage Specialist plays a crucial role in connecting individuals in need of mental health and behavioral support with appropriate CBHC services. The position will be responsible for assessing client needs, and providing initial triage and referral services. This position requires a compassionate and empathetic individual with excellent communication skills and a strong understanding of behavioral health services. The position includes afternoon and evening hours (4-8 pm) as well as some weekend hours.
ESSENTIAL FUNCTIONS (*):
- Conduct outreach activities:
- Collaborate with community organizations, schools, healthcare providers, and other relevant stakeholders to promote awareness of our services and establish partnerships.
- Develop and implement strategies to reach underserved populations and marginalized communities.
- Perform program and front desk coverage and initial triage:
- gather information about the individual's mental health and behavioral needs.
- Manage check in and check out process.
- Answering phone calls from clients and community partners
- Evaluate the urgency and severity of each case to determine appropriate levels of care and intervention.
- Collaborate with a multidisciplinary team to ensure timely referrals to appropriate CBHC services.
- Provide information and referral services:
- Educate individuals about available CBHC services, treatment options, and community resources.
- Assist individuals in navigating the mental health system, including making appropriate referrals and scheduling appointments.
- Maintain accurate and up-to-date records of client interactions and referrals made.
- Coordinate care and follow-up:
- Collaborate with internal and external providers to ensure a seamless transition of care for clients.
- Follow up with individuals to monitor their progress and provide additional support as needed.
- Provide crisis intervention and de-escalation techniques when necessary.
- Documentation and reporting:
- Maintain thorough and accurate documentation of client interactions, assessments, and referrals in accordance with organizational and regulatory requirements.
- Prepare regular reports on outreach activities, client engagement, and outcomes for management and funding purposes.
QUALIFICATIONS/ABILITIES:
- Bachelor's degree in psychology, social work, counseling, or a related field preferred.
- Previous experience in outreach, case management, or related roles within the behavioral health field.
- Knowledge of mental health disorders, treatment modalities, and community resources.
- Strong interpersonal skills with the ability to establish rapport and build trust with diverse populations.
- Excellent communication skills, both written and verbal.
- Ability to work independently and as part of a team in a fast-paced environment.
- Proficient in using computer systems and software for documentation and reporting.
- Bilingual (Brazilian Portuguese/English) and/or certification as a Recovery Coach an asset
