The Care Transition Specialist at Martin Luther King, Jr. Community Hospital plays a critical role in ensuring seamless patient care transitions by coordinating communication between healthcare providers, payors, and patients. Key Responsibilities:
- Develop and implement safe, effective, and timely discharge plans.
- Collaborate with the interdisciplinary team to re-assess and adjust care progression and transition plans according to patient clinical condition.
- Facilitate palliative or hospice care when needed.
Requirements:
- Bachelor of Science degree in nursing (preferred) or Associate's degree in nursing.
- Minimum one to three years of hospital or related experience required.
- Able to navigate outside provider networks (health plans, IPA's, and FQHC's).
