Duties and Responsibilities:
- Assess patient healthcare needs and goals, targeting physical, functional, psychosocial, environmental, and financial status.
- Develop and implement safe, effective, and timely discharge plans.
- Collaborate with the interdisciplinary team to re-assess and adjust care progression and transition plans according to patient clinical condition.
- Advocate for patients with payers and/or IPA's to ensure effective care progression and transition plans.
Qualifications:
- Bachelor of Science degree in nursing (preferred) or Associate's degree in nursing.
- Minimum one to three years of hospital or related experience required.
- Able to navigate outside provider networks (health plans, IPA's, and FQHC's).
