Responsibilities
- Supervise and direct a team of clinical staff to ensure effective patient care transitions.
- Collaborate with other departments to identify opportunities to improve post-discharge services and reduce readmissions.
- Establish and monitor management metrics: quality, outcomes, productivity of staff work, financial, case volume/mix, and provide scheduled reporting to the Director of Care Management.
