Job Summary :
The Hospital Liaison plays a vital role in bridging the gap between hospitals and our skilled nursing facility (SNF). This individual is responsible for developing and maintaining strong relationships with hospital case managers, discharge planners, and physicians within the Beaumont Health System. The Hospital Liaison will ensure that patients, families, and referral sources have a clear understanding of the services and benefits our SNF provides, ultimately driving census growth and optimizing patient care.
Key Responsibilities :
- Develop and maintain relationships with hospital discharge planners, case managers, social workers, and physicians to generate referrals.
- Assess potential patients for clinical and financial eligibility for admission to the SNF.
- Collaborate with the admissions and nursing teams to ensure seamless transitions from hospital to facility.
- Educate patients, families, and hospital staff on the services and benefits of our skilled nursing facility.
- Conduct facility tours for prospective residents and their families.
- Maintain accurate documentation of referral sources, patient inquiries, and admission outcomes.
- Track and analyze referral trends to identify opportunities for growth.
- Work closely with the marketing and admissions team to meet census goals.
- Represent the facility in community and professional networking events to strengthen partnerships.
