Salary Range $20.00 - $24.00 Per Hour

ESSENTIAL DUTIES AND RESPONSIBILITIES

QUALIFICATIONS

EDUCATION and / or EXPERIENCE
Bachelor's degree in Human Services, Nursing, or related field required; Master's degree in Human Services or Nursing preferred, and a minimum of one (1) to three (3) years related care management experience with persons with disabilities and / or chronic illnesses.
CERTIFICATES, LICENSES, REGISTRATIONS
Must have a valid NYS driver's license with an acceptable driving record according to Mary Cariola Center guidelines.
