CHILDREN'S HEALTH HOME CARE MANAGER

Salary Range $20.00 - $24.00 Per Hour

Pay within this range is based on experience and education

ESSENTIAL DUTIES AND RESPONSIBILITIES

Job Specific:

• Understands the medical, psycho-social, educational characteristics and needs of the population served by the Agency
• Thrives working independently and on a team
• Provides outreach and enrollment, develops Plans of Care and conducts continuing care management services with children and families
• Coordinates, identifies, and secures services relevant to the Plan of Care
• Completes documentation with a high level of quality and detail to meet and exceed Health Home, and Department of Health requirements
• Participates actively in trainings, meetings, and functions at the agency and in the community
• Provides quality direct Care Management services to children and families in home, school, and community settings
• Uses a strength-based approach to arrange for, educate about, and manage the coordination of medical care and other services as needed
• Provides on-call availability to Health Home Program as assigned
• Interfaces and networks with partnering Health Homes, providers, and agencies
• Must obtain certification to administer CANS-NY within 90 days of employment; certification must be continuously maintained while in the position
• Other duties may be assigned.

EDUCATION and/or EXPERIENCE

Bachelor’s degree in Human Services, Nursing, or related field required; Master's degree in Human Services or Nursing preferred, and a minimum of one (1) to three (3) years related care management experience with persons with disabilities and/or chronic illnesses.

CERTIFICATES, LICENSES, REGISTRATIONS

Must have a valid NYS driver's license with an acceptable driving record according to Mary Cariola Center guidelines.
