Role Description
In partnership with Mary Free Bed Rehabilitation Hospital, Mary Free Bed at Home is an accredited leader in home health, delivering industry-leading therapy and nursing expertise throughout the West Michigan service area. This position supports patients in Grand Rapids and surrounding areas, requiring a deep understanding of patient needs and a strong ability to develop and implement effective care plans.
Responsibilities
- Collaborate with interdisciplinary teams to provide management and delivery of patient care plans, prioritizing individualized care and exceptional outcomes.
- Develop and maintain strong relationships with patients, families, and healthcare professionals, fostering open communication and trust.
- Utilize clinical knowledge and social work expertise to identify and address patient needs, promoting seamless transitions between care settings.
- Work collaboratively with therapists, nurses, and other healthcare professionals to ensure comprehensive and coordinated care.
- Provide emotional support and guidance to patients and families, addressing psychosocial challenges and facilitating smooth transitions.
- Maintain accurate and detailed records, ensuring compliance with regulatory requirements and providing timely updates to multidisciplinary teams.
Requirements
- Masters degree in Social Work from a graduate school accredited by the Council of Social Work.
- 1-2 years postgraduate social work experience in a healthcare setting.
- Current license as a Medical Social Worker issued by the state in which you work, maintained in good standing.
- Valid driver's license and ability to spend approximately 20% of your day driving to/from patient locations.
- Strong time management and scheduling skills, with the ability to meet critical deadlines for charting.
- Demonstrated experience with tablets, mobile phones, and EMR software, with a high level of proficiency.
