Care Navigator (CDCA/APS) Job Description

Under the direction of the Clinical Manager and/or Lead Counselor, the Care Navigator is responsible for substance use and mental health case management and care coordination for people who have been referred to Maryhaven for services. The Care Navigator may also consist of peer recovery services if the person has proper state credentials and experience. This role serves as an important function at Maryhaven, which may include outreach to community organizations and partners, as well as hospital systems and points of linkage. The employee must be able to work independently but also possess strong team-oriented skills. The applicant may also be responsible for transporting clients in Maryhaven vehicles for appointments and different locations for services.
