Job Summary:
Provides appropriate oversight and guidance to individuals, facilitating greater independent functioning within the community.
Essential Functions:
Provides daily instruction, direction and guidance to residents that is consistent with their individual needs.
Submits and completes manager’s monthly summary by required date.
Develops Residential Habilitation Goal Plans by required dates and completes documentation.
Assists consumer to develop weekly menu, shop, and assist in proper food storage.
Facilitates higher-level ADL skills (i.e. cooking, nutrition, meal planning, shopping, travel training, household maintenance and chores and financial responsibilities such as bill paying, banking and budgeting).
Identify main physical plant repair needs and ensures follow-up is completed, in a timely fashion.
Maintains current documentation as required (i.e., DDP, medical paper, medication schedules, financial records and receipts, resident goal plans, wages and financial status, fire drills as required by regulation).
Arranges medical appointments for consumers and document results.
Implements medication administration plan with consumers.
Monitors consumer’s medication on weekly basis, documenting this and alerting program nurse of any issues noted.

Pay: $21.00 - $22.00 per hour
