Compensation & Benefits
- Salary: $75,000 - $80,000 annually
- Paid Time Off: 3 weeks (2 weeks 6-7 federal holidays)

- Assist patients in securing and maintaining stable housing, including support with housing applications, financial assistance programs, and landlord negotiations.
- Connect patients with local, state, and federal housing resources, such as HUD programs, Section 8, and emergency rental assistance.
- Work collaboratively with healthcare teams, case managers, and social workers to integrate housing support into care plans.
- Educate patients on financial and economic resources to improve overall health and stability.
- Provide regular updates to the care te am regarding patient needs, barriers, and progress.
- Bachelor’s Degree (preferred), Associate’s Degree (required)
- 2 years of experience in housing support services (required)
- Strong knowledge of federal and state programs, including disability benefits, food assistance, and IHSS
- Bilingual candidates encouraged to apply
