The Care Coordinator Manager will coordinate and facilitate comprehensive care management services for patients enrolled in the ACO, including full assessments, care planning, and ongoing care coordination. Identify and coordinate access to healthcare resources, community services, and support programs to address patients' medical, social, and behavioral health needs. Collaborate with interdisciplinary teams, including physicians, nurses, social workers, pharmacists, and care coordinators, to develop integrated care plans and care coordinate services across disciplines.
REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES :
Registered Nurse (RN) or another licensure (e.g. LPN, Licensed Clinical Social Worker- LCSW) required.Bachelor's degree in nursing, social work, or related field (master’s degree preferred).Previous experience in leadership skills of a care management team, care coordination, or case management within a Healthcare setting, preferably in an accountable care organization, health system, or community health organization.Strong clinical assessment skills, with the ability to conduct comprehensive health assessments, develop care plans, and implement evidence-based interventions to address patient needs.Knowledge of healthcare delivery systems, reimbursement mechanisms, and regulatory requirements related to care coordination, care management, and value-based care models.Proficiency in using electronic health records (EHR), care management, systems, and other healthcare technology tools to document patient information, track care plans, and communicate with interdisciplinary teams.Excellent communication, Interpersonal, and teamwork skills, with the ability to collaborate effectively with patients, caregivers, healthcare providers, and community resources.EDUCATION REQUIREMENTS
