As an Emergency Department Social Worker at Mason General Hospital, you will provide essential psychosocial support to patients and their families in a fast-paced, high-stress environment. Your role will involve assessing and addressing the emotional, social, and practical needs of individuals in the emergency department, facilitating crisis intervention, and coordinating with multidisciplinary teams to ensure comprehensive patient care.
Duties and Responsibilities :
Patient and Family Support :Conduct psychosocial assessments to identify patient and family needs.Provide crisis intervention, emotional support, and counseling to patients and their families.Assist with emotional, social, and practical aspects of patient care, including coping with trauma, grief, and loss.Resource Coordination :Connect patients and families with community resources, support groups, and other social services.Facilitate discharge planning, including arranging for follow-up care, referrals, and ensuring a smooth transition from the hospital to home or other care settings.Assess and assist with Social Determants of Health needs of the patient.Multidisciplinary Collaboration :Work closely with emergency department physicians, nurses, and other healthcare professionals to develop and implement patient care plans.Participate in case conferences and contribute to team discussions about patient care.Crisis Management :Address immediate safety concerns, such as potential for self-harm or harm to others.Manage cases involving abuse, neglect, and other significant social issues.Documentation and Compliance :Maintain accurate and confidential records of patient interactions, assessments, and interventions.Ensure compliance with hospital policies, legal requirements, and ethical standards.Education and Advocacy :Provide education to patients and families about their rights, available resources, and the healthcare system.Advocate for patients' needs within the healthcare system and the community.Required Education and Experience :
Masters Degree in Social Work (MSW) from an accredited institutionMinimum of 2 years of social work experience. Healthcare or Emergency setting preferred.Minimum of 3 years of experience in hospital or clinic coding.Required Licenses, Certifications and / or Registrations :
Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.Required Knowledge, Skills and Abilities
Strong interpersonal and communication skills.Ability to work effectively in a high-stress, fast-paced environment.Knowledge of community resources, mental health issues, and crisis intervention techniques.Proficiency in electronic medical record systems and other relevant software.Requires sufficient English grammar and writing skill to read and understand technical data, and to document work, prepare reports, and develop procedures.Requires sufficient human relations skills to exchange technical information with medical providers and hospital support staff, take part in in-service education, and work as part of a collaborative team.Requires the ability to carry out the duties and responsibilities of the positionPI260387251
