As an LPN/RN, you will carry out our Mission of Love by assisting our residents in reaching or attaining the highest potential in their physical, mental and psychosocial well-being.
Our LPN/RN is responsible for the following:
- Provides Practical Nursing care for our residents in a safe, caring, timely and appropriate manner to assist each resident in attaining their highest potential in their physical, mental, and psychosocial well-being, within the scope of practice and Masonic Village policies and procedures.
- Monitors nursing care by visiting residents routinely to ensure continuity and continual evaluation of care as related to the IDCP and physician's orders. Monitors resident's condition and nursing care needs, assures that medications, treatments, and care are provided as ordered or specified on the IDCP. Maintains appropriate documentation of nursing observations, including follow-up documentation in the medical record in accordance with acceptable
medical standards of practice. Prepares equipment and assists physician during rounds or special treatments and examinations of residents if needed. Provides direct resident care and transports residents to activities and appointments as time permits. Performs glucometer readings, EKG's and other testing as needed. Assists with direct resident care including bathing, feeding, making beds, toileting and transporting residents to appointments and activities. - Actively participates in change of shift report concerning condition of residents and any unusual occurrences. Maintains accountability for counting narcotics at change of shift with another licensed nurse and reports any discrepancies upon first notice.
- Observes residents and answers call bells in a timely manner to respond to residents' care needs. Reports observations, changes, unusual conditions or reactions to supervisor or appropriate discipline in a timely manner. Takes appropriate action as requested or required by the situation. Notifies family of change in residents condition or hospitalization according to Masonic Village policy.
- Assist to identify resident needs and strengths. Assists in the development of the plan of care. Participates in resident care conferences as needed. Implements the plan of care and oversees care provided by assigned nursing personnel.
What You Have:
- Graduation from an accredited school of nursing as a Licensed Practical Nurse or Registered Nurse.
- Current LPN or RN licensure by the PA State Board of Nursing.
- CPR certification (prior to or upon employment).
- Previous experience in long-term care preferred.
- Basic computer knowledge required.
- Effective oral and written communication skills required.
