What You'll Do:
As a Social Worker you will carry out our Mission of Love by assisting patients, families and caregivers with emotional and practical issues while receiving care through the hospice or home health.
Our Social Worker is responsible for the following:
- Assures that social service activities are integrated and coordinated within the hospice and home health departments.
- Performs psychosocial, bereavement and safety assessments on all patients and families assigned. Develops specific, measurable, realistic plans and objectives with the goal of quality of life and safety in mind.
- Serves as a resource to patients and families for questions and emotional support.
- Demonstrates knowledge of clinical social work and cultural competency in your work.
- Identifies and addresses psychosocial and safety needs of patients and families.
- Communicates and collaborates appropriately and clearly to all members of the department and facility/referral source teams.
- Facilitates hospice admission meetings and demonstrates competency and proficiency in explaining Medicare hospice regulations, advanced care planning, and hospice education for patients, families, and referral sources.
- Demonstrates the ability to be flexible, organized and function under stressful situations.
- Makes referrals to outside agencies or groups as needed.
- Maintains current knowledge of Medicare Conditions of Participation for social work services within Home Health and Hospice.
- Documents in accordance with Medicare regulations and department standards.
- Is an active member of the team meetings.
- Participates in the on-call hospice supportive services rotation.
- Carry out QAPI roles and responsibilities as assigned.
- Participates in committees and projects as assigned.
What You Have:
- Graduated from a four year college or university with a bachelor's degree in social work.
- MSW required in accordance with Medicare Conditions of Participation for Hospice.
- Personal computer experience required
- Hospice, Home Health or Medical Social Work experience preferred.
- Maintains a current PA driver' s license and insurance on personal vehicle
